This will begin our first step with enrollment of students for the 2018-19 school year. Please make any corrections to the form, sign, and return it. Some of the corrections could be phone number, address, and pick-up list. Please make sure the phone numbers for individuals on the pick up list are correct. You may add or remove individuals on the pick up list from this form by crossing through them. If your child is being moved due to redistricting, please still sign and return the form and we will work together as an elementary team and send them to each school. All forms must be returned by Tuesday, May 8th.
